Personal Identification Card

WHO ARE YOU ???

A number of months ago, I had the occasion to spend time in a hospital and then in care. As a result, I learned a number of things. This section has some very important information of some of those things that I'd like to share with you.

Never Leave Home Without Having:

Your Name, Address and Phone #

Name, Address and Phone # of Next of Kin

Your Doctor's Name, Address and Phone #

A List of Your Allergies

Your Doctor's Prescription Information

Extra Prescription Instructions

Travel Kit of Medications

WHY?
If you decide to go for a walk, you could be struck down by a car, a falling tree, or suffer a heart attack and become unconscious.

Your Name, Address and Phone #
This personal information provides the police and paramedics with information on who you are and where you live.

Name, Address and Phone # of Next of Kin
This information allows the emergency responders to verify who you are and enables them to advise family members that you've been taken to the hospital. The 'next of kin' can then alert them of the particulars they feel the responders should know about you.

Your Doctor's Name, Address and Phone #
Having these details allows hospital staff easier access to your doctor should they require more information about you.

A List of Your Allergies
Your food and drug allergies should be clearly listed somewhere so that you will not be given some food or drugs that will cause an unnecessary adverse reaction.

Your Doctor's Prescription Information
The hospital staff need to know exactly what medications have been prescribed for you, for what reason, taken at what dosage, and how often, in order for them to be better able to treat you.

This following section is very important, should you arrive at a hospital that does not have staff experienced with Parkinson's Disease.

Extra Prescription Instructions
You should have on hand, extra details that are pertinent as to the times you take your medications and, we'll say, what foods you avoid when doing so. For example, if you are taking Sinemet CR, you would want the hospital staff to know that this drug should be taken 30 minutes before or after meals and never with meals that have a large amount of protein!
So, how best to do this?

YOU CAN HAVE IT ALL IN A .....…

Travel Kit of Medications
Whether you are only going for a walk to the store, or going on an extended trip, it is a good idea to carry a 'Travel Kit of Medications'. This 'kit' should include, at least, 4 to 5 days of ALL your medications and be accompanied with something that includes ALL of the above information. What could that be?

Personal Information Card
I am suggesting that you could include ALL the above information on ONE card to be inserted into your Kit'. Sample below.

Personal Information Card
The holder of this card has Parkinson's Disease (and name any other afflictions)
Full Legal Name: .
Address & Phone # .
Next of Kin Info.:

Son: (name) and (phone #);
Daughter: (name) and (phone #)

Name of Medical Carrier and Medical Number:
Prescriptions (name of drug, dosage and times taken, and what the drug is for):
e.g.:

Examples:
Sinemet CR, 200/50 for Parkinson's Disease
@: 7 a.m.,11 a.m., 2 p.m., 5 p.m., 8 p.m.
Comtan 200, for Parkinson's Disease
@ 7 a.m.; and 1/2 Comtan @ 8 p.m.
Name of Pain medication 5 mg and 10 mg as required (for pain associated with ____________)
Other

Instructions if necessary): Sinemet CR and Comtan 200 should be taken either 1/2 hour before or 1/2 hour after meals and NEVER with a great amount of protein!
Allergies (drugs and food): Codeine, Demerol, Morphine, MSG
Name of Physician, address, phone # and signature: Signature of Physician Office Stamp (above)
** Always carry 4 or 5 Days of Medication! **
